The above file is provided as a downloadable Adobe Acrobat PDF file type. You will need Adobe Acrobat Reader to view these files. Adobe Acrobat Reader is free software for viewing and printing Adobe Portable Document Format (PDF) files on all major hardware and operating system platforms. This is a very useful add on program that many people already have installed on their computer. If you need to get the free Adobe Acrobat Reader program please use this link: click here to get Adobe Acrobat
The Certificate of Insurance can be submitted by either of the methods below. The form has been configured to be interactive, allowing you to fill it out electronically, if desired, or printed and filled in manually.
A. Download the form, open it with Adobe Reader, print it, complete it as necessary, and fax it to the number shown.
B. Download the form, open it with Adobe Reader, fill it in using Adobe Reader, and save the filled in form. Email the filled in form to the address shown on the form. You can include a short message such as “Attached is a Request for Certificate of Insurance” and click send. (don’t forget to attach the completed form)
There is a statement that says “Is Certificate Holder requested to be named as an additional insured?” If you only need to show proof of insurance, circle No or click the button for No. If the venue needs to be named as an additional insured, circle Yes or click the button for Yes.
Please remember, I do not obtain the Certificate of Insurance for individual clubs. If you want to have the certificate mailed to the venue, fill in that area. If you want it faxed, fill in that area and be sure to include the persons name to which you want the certificate faxed.
